The really necessary procedures account for
about one in six births, and
therefore any signi?cant increase over this ratio is probably due to cultural factors.
Decisions
Ultimately, a Caesarean section is a medical procedure
with a complex set of conditions and possible complications. It has a long history, but until the modern era,
it was an act of desperation
that meant almost inevitable
death for the mother. On the other
hand, the Nordic states have
relatively low rates . Incontinence has
a major impact on quality of
life, and this is a very real concern for many mothers.
In Finland, C-sections can
also be performed when the
mother has a fear of childbirth. There are some
indications that a C-section
done as little as a few days early may increase the risks of
complications signi?cantly.
On the other hand, C-sections may reduce the incidence of urinary incontinence
among mothers, as the pelvic

such as uterine rupture or

A C-section may also be
performed electively. This
is particularly common in the
case of women who have had
previous C-sections, because
there is a higher risk of uterine rupture if they attempt
vaginal delivery.
In addition, there has been
a massive rise worldwide
in the number of C-sections

. Nations
with universal, high-quality
healthcare often have quite
low rates. The obstetrician in charge of the delivery will make the decision
in such a case, with the consent of the mother. the
highest is Denmark with 21.4
per cent, but then we have
Sweden at 17.3 per cent, Norway with 16.6 per cent, and
Iceland with 15.6 per cent.
It is clear that the rate of
C-sections is not well correlated with the availability of healthcare. With
modern medical technology,
however, the risks of a C-section are fairly low.
A C-section is medically
indicated when vaginal delivery may pose risks to the
mother or child. In the interests of
mother, child and the family
as a whole, a C-section may be
recommended in these cases.
Doctor Anthony Falconer,
President of the Royal College
of Obstetricians and Gynecologists in the UK, has written,
?We must remember that caesarean sections are clinically indicated to improve safety
and outcome for mothers and
babies in well de?ned situations. This means that women
who should be having C-sections are not receiving them.
On the other hand, C-section rates have been rising
in many parts of the world,
with many being performed
without being medically necessary. In some cases, it is a
clear-cut medical decision, but
in others there is a strong element of personal choice.. It is important for
expectant parents to learn
and understand the risks and
bene?ts, and weigh them carefully in deciding whether to
have one. The Mediterranean
countries score highly, with
38.2 per cent in Italy, 34.0 per
cent in Portugal and 25.9 per
cent in Spain. The reasoning is that severe psychological stress can
be very harmful for the mother and lead to problems down
the road, notably post-partum
depression. The World Health Organisation (WHO) has found
that almost half of all births
in China are delivered by Csection, with high rates elsewhere in Asia as well.
C-sections are also popular in Latin America, with
45.9 per cent of births in
Brazil.
Iran is another high scorer, with a 41.9 per cent rate in
2008.
Rates vary widely across
Europe. Recent advances in
medical science have made
the procedure much safer and
for most women the complications of this operation are low.?
Rates
The number of C-sections
done in Finland is relatively
low for a country with good
access to healthcare. If there
is a known risk for complications, an elective C-section may be scheduled. This is done on
the grounds of scheduling or
to reduce the risk of urinary
incontinence.
Risks and rewards
Performed under proper surgical conditions by a competent
obstetrician, the C-section is a
relatively safe procedure, with
a mortality rate of less than
0.05 per 1,000 in most countries for the mother.
The mortality rate for the
baby is higher, albeit because
many C-sections are performed when there is a problem with the pregnancy or the
child?s development. However, even with non-medically indicated C-sections, there
is a higher rate of complications than there are with vaginal births, especially if the
C-section is performed before
the pregnancy is full-term, i.e.

?The in?uence smoking
has on the fetus. Babies with these symptoms are often unusually tearful,
overwrought and restless. For example, in Somalia, the caesarean
section is considered a dangerous procedure, and some
of the women immigrating
and giving birth in a new environment might consider the
procedure to be dangerous
in Finland as well. This leads
them to deliberately restrict
their diet, since scarce eating
also results in the fetus being
smaller than usual.
Fear of childbirth
as the bottom line
A public health nurse from the
maternity- and child welfare
clinic of Laajasalo health center, Raija Saavalainen-Kourula, emphasises that the main
reasons for women smoking
during pregnancy are more
usually strong addiction and
stress-management than the
thought of reducing the size
of the fetus. Also
laboratory and X-ray services are available. Fear of
childbirth is considered a valid justi?cation for granting a
caesarean section in Finland.
Extensive changes
for finding solution
Maternity- and child welfare
clinics offer a wide range of
support to pregnant women.
For example, the consumption of intoxicants and smoking are discussed right from
the ?rst appointment, and
from then on clinics try to actively support and encourage the family in improving
their life choices for the baby?s health. MEDICAL CENTRE WITH

Our versatile medical centre in southern Helsinki offers you medical services
in several care areas. She has, however, heard about the phenomenon through her colleagues.
Saavalainen-Kourula also believes mothers prefer to share
their views in discussion forums rather than in maternity clinics.
Luckily, the phenomenon
of women starting to smoke
to reduce the baby?s size is only marginal, Gissler and Saav-
alainen-Kourula say. It may also cause chronic lack of oxygen, low birth weight and symptoms of withdrawal from nicotine. The in?uence that smoking has on

same as giving birth only one
week earlier than the expected date of delivery. However,
the work of maternity clinics
is still vital, especially due to
the increasing fear of childbirth, which is the ultimate
reason for the phenomenon.

have increased notably during the last two years, ac-
cording to a study by THL,
the Finnish National Institute for Health and Welfare.
Over 14,900 fertility treatments were initiated in 2011.
The number of treatments
with donated sperm has in-
creased by 25 per cent, according to the study.
Despite the increase in
sperm donation, fertility
clinics must rely on foreign
sperm banks, mostly in Denmark, besides Finnish donors. The law also regulates
the use and storing of donated sex cells. In 2010, 95 per
cent of all in-vitro fertilisation treatments were performed in private clinics,
with only two units operating
in the public health sector, according to the report by THL.
The requirements for being eligible for sperm donation are strict in Finland,
as results of fertility treatments are monitored by THL.
According to the Family Federation, the donor must be

no known family history of

So maybe Copenhagen or Stockholm will have

Brenda ends.
J I M M Y L I TA R D O
A LITTLE less than a year ago,
two girls from New York decided to introduce the taste
of Brooklyn to Helsinki residents by opening their own
cafe.
Being daughters of an entrepreneurial family, Sharron and Brenda Todd dreamt
of owning their own cafe
since childhood. Richter became
famous from the ?fth season of American Top Chef
as the ?rst Finnish person
to attend the show.

Brooklyn Cafe,. That?s why we try even
more,. We?re also expanding to a bakery, for which
we are looking for a proper
space,. . As the sisters grew up, they started to
make real plans for ful?lling their dream, but carrying them out seemed to take
forever.
One night, the sisters
started to think about their
future and realised that they
were afraid of having to spend
the rest of their lives being
scared of the risks of opening
their own business. Sharron explains.
Like Sunday morning
The two sisters wanted to
serve food that represents
and tastes like their world.
The sweets in the shop are
baked using Brenda?s recipes from childhood, and the
cafe?s specialty bagels have a
unique homemade New York
?avour.
The Todds wanted their
place to feel almost like a living room for their customers.
They wanted the place to be
?lled with the smell of coffee and joyful conversation
so that, as they describe it,
it would always feel like Sunday morning.
Not surprisingly, the comments on the café?s Facebook
page praise the place for its
warm and cosy atmosphere,
describing it as a smile-maker and the saving of the day.
Some even say it has the
best service in town, which
isn?t hard to believe after
chatting with the adorable

the younger of the two.
The sisters admit that
they were a little scared
about opening their own
business in the beginning.
However, their genuine
love and passion for their
idea kept them believing in
themselves.
?We?re doing something
that truly represents us and
our visions, and who we are.
We?re not just pleasing people by doing something su-
per?cial, because when you
do things you don?t actually believe in, you?re bound to
fail,. 14 NOVEMBER 2012

washing it down with a sip of
vodka instead of the traditional ice-cold milk. Black sausage, lingonberry jam and cold milk:
it cannot get any more Finnish than that!. Its black colour may be
quite uninviting, and might

In reality, after a ?rst encounter with the black sausage, it is likely to become
less of an unknown, bizarre
black thing, but rather an appetising snack.
Black sausage is made by
mixing pork, pig?s blood (the
source of the black colour),
crushed rye and ?our, after
which it is stuffed into the

The Elephant man
Set in 1966, the film tells the
story of a pirate radio station,
aired from a boat near the coast
of the United Kingdom. In the
film, a surgeon, played outstandingly by Anthony Hopkins, finds
a heavily disfigured man at a

into his hospital for care. In those
days, the BBC radio didn?t play
rock and pop music on the
airwaves, so many music fans
set up their own radios, in boats
anchored on international seas
and aired the music from there.
The film depicts one of those
boats and its crew, as the hosts
talk dirty and play unlicensed
music to Britain. Starring:

Now, as the days are getting increasingly shorter and
we?ve just had our ?rst snowfall, I ?nd myself mentally preparing for the months

and somewhat apprehensive
about, but I?m absolutely fascinated to see how I?ll cope
with such a different winter
experience, and I can?t wait
to experience the way Helsinki adapts over the coming
